Show a current state issued picture ID.
The fee is $16 for the first document and $8 for each copy for each individual.

Records are available back to 1977. Older records must be requested from the Office of Vital Records and Statistics, 288 N 1460 West, P.O. Box 141012, Salt Lake City, UT 84114-1012. The time frame is 3 to 4 weeks. These certificates can be expedited in a 24 hour time period for a fee of $30.
