patients quit tobacco use?
Even brief advice to quit using tobacco by a healthcare provider is proven to motivate quitting and significantly increase the chances of quitting success. If the healthcare provider also provides patients with quitting medications and refers patients to cessation support programs, their chances of quitting success are even higher.

The TRUTH Network Healthcare Provider Guide: Treating Nicotine Dependence in Patients is a tool designed to help healthcare providers assist their patients who use tobacco. It explains the 5 A's of tobacco control, available pharmacological therapies, statewide cessation programs and fax referral to the Utah Tobacco Quit Line. The Public Health Service Guideline summary is also included. Two pamphlets to share with patients accompany the guide, one for orienting patients to the Quit Line (when they fill out a fax referral form) and another to motivate patients who are not ready to quit.
The fax referral form is a way to sign-up consenting tobacco users to receive a phone call from the Utah Tobacco Quit Line (Quit Line). All you have to do is hand them the fax rererral form and then have you office fax it off. Within a few days professional quit coaches will contact your patient. Get your fax referral form.
